I have enough parts to build a Picco P45. The parts I have are for the first generation P45 where the head button sits on top of the liner. The case has no fins on the exhaust stub and it has a drum intake. I believe all Picco after the first generation has the head button intrude into the liner.

It's been almost 40 years and I cannot remember if there is a head shim between the head button and the liner. If there is one, what is the correct thickness? Will the head shim for the late model 45 fit?

Yes there is a shim for setting head clearance and sealing the combustion chamber. I don't have any specs on it but if you were to post the OD/ID of the head where it sits on the cylinder liner I'm sure someone will have one. All my Picco's are lat model versions and i don't recall if they are the same.
 
I have some Picco head shims (0.1mm & 0.2mm) which the ID is 21.8mm. The bore size is nearly no changed since 80's models.
0.1mm is $1.60 each and 0.2mm is $1.75 each plus shipping.
