unless you want some eye candy, the du-bro and octura are readily available. each costs about $20. the octura offers more setback, and a little more height adjustment. also, the octura pivot block is aluminum-not nylon-, and has pivot pin bushings.

I can vouch for Capt Vic's mounts. They are very nice,
