Where should the flanged end of the bushing be? Hanging out the back or flush with the strut? Somewhere in between?

I would leave it just behind the back of strut. If you recess it it may bind. Just make sure you leave enough room for cable shrink under load from drive dog to bushing. I usually flush my stuffing tube to the back of strut and then the bushing rides up to that. If you use a AB strut, you could recess it. It has a tapered indent.
